Build the matrix on risk scenarios
Define required skills by scenario, not only by policy chapter:
- Confined space entry
- Lockout/tagout
- Contractor induction
- Incident response and escalation
Add proficiency levels
Track proficiency levels for each role:
- Awareness
- Assisted execution
- Independent execution
- Coach level
This gives managers better deployment decisions for shift planning and temporary coverage.
Tie learning to incident data
If incident trends rise in a task category, trigger retraining automatically for affected roles. Your LMS should support this as a rule-driven workflow.
Audit training effectiveness quarterly
Review more than completion rates:
- Time to competency
- Skill decay after 90 days
- Incident recurrence post-retraining
